Apply directKey Responsibilities:
- Coordinate, complete, and submit accurate and timely MDS assessments in accordance with CMS guidelines.
- Ensure compliance with the RAI process, Medicare, Medicaid, PDPM, and all state and federal regulations.
- Develop, update, and coordinate comprehensive resident care plans with the interdisciplinary team.
- Conduct Medicare meetings, Triple Check meetings, and utilization review meetings to ensure reimbursement accuracy.
- Review clinical documentation to support appropriate coding, case mix, and reimbursement.
- Perform routine chart audits to ensure documentation accuracy, regulatory compliance, and survey readiness.
- Monitor assessment schedules and ensure all OBRA, PPS, IPA, and significant change assessments are completed timely.
- Collaborate with nursing, therapy, dietary, social services, and physicians to ensure comprehensive resident care.
- Educate and mentor clinical staff on MDS documentation requirements, care planning, and regulatory changes.
- Analyze quality measures and participate in QAPI initiatives to improve clinical outcomes.
- Assist during state surveys and respond to regulatory inquiries related to the MDS process.
- Maintain confidentiality while demonstrating professionalism and excellent communication with residents, families, and staff.
Qualifications:
- Current RN or LPN license in TN.
- MDS certification (RAC-CT preferred).
- Minimum of 2 years of MDS experience in a skilled nursing or long-term care setting.
- Strong knowledge of PDPM, Medicare reimbursement, OBRA regulations, and CMS guidelines.
- Excellent organizational, analytical, communication, and time management skills.
- Proficiency with electronic health records (EHR) and MDS software.
