- Position Summary
The Social Services Director is responsible for overseeing and coordinating the social services department, ensuring residents receive the emotional, psychosocial, and support services needed to achieve and maintain the highest possible quality of life. This role works closely with residents, families, interdisciplinary team members, and community resources to provide comprehensive support throughout the resident's stay.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for residents.
- Develop, implement, and update individualized care plans.
- Assist residents and families with adjustment, discharge planning, and community resource referrals.
- Advocate for residents' rights and promote resident-centered care.
- Coordinate family meetings and provide support and counseling as needed.
- Ensure compliance with federal, state, and facility regulations regarding social services.
- Collaborate with the interdisciplinary team to address resident needs and improve outcomes.
- Maintain accurate and timely documentation.
Qualifications
- Bachelor's Degree in Social Work or Healthcare.
- Previous nursing home or long-term care experience required.
- Strong knowledge of psychosocial assessment, care planning, and discharge planning processes.
- Excellent communication, organizational, and interpersonal skills.
- Ability to build positive relationships with residents, families, and staff.
