• Complete MDS Assessments accurately
• Ensure timely Assessments and Transmittals
• Complete individualized Care Plans
• Conduct Care Plan Meetings with residents and families
• Perform other assigned tasks as needed
Qualifications:
• Current RN or LPN license in IL (required)
• Previous MDS experience
• Flexible with time and able to prioritize effectively
• Knowledge of AHT and PCC systems
• Understanding of Medicare ARDs
• Strong attention to detail
