Home Health Nurse - RN

Home Health RN Case Manager needed in North County San Diego CA. (Oceanside, Carlsbad, Encinitas, San Marcos, Vista). You will coordinate and advocate for patients, ensuring comprehensive and individualized care is provided within their homes. You will develop and implement care plans, collaborate with healthcare providers, and educate patients and families on their conditions and treatment options.

Responsibilities:

• Patient Assessment and Care Planning:
• Conducts assessments determining patient's needs & develop a plan of care.
• Collaborates with patient, family, and healthcare providers establishing goals.
• Ensures care plans meet patient needs, addressing physical, emotional, & social needs.
• Care Coordination and Supervision:
• Coordinates delivery of care, delegating tasks (physical therapists, home health aides).
• Monitors the patient's progress and makes adjustments to the plan of care as needed.
• Supervises/supports home health team, ensuring quality & adherence to protocols.
• Communication and Documentation: EMRs (WellSky, Kinnser, Oasis, MS Office Suite)
• Communicates with patients, families, physicians, providers to coordinate care.
• Maintains documentation of patient assessments, care plans, and progress.
• Participates in team mtgs & case conferences sharing info. & collaborate on care.
• Patient and Family Education: Explaining services, care and processes.
• Provides education to patients/families about treatments, & home care procedures.
• Addresses questions/concerns, offering support/guidance throughout care process.
• Serves as a patient advocate, ensuring their needs and preferences are considered.
• Identifies and facilitates access to community resources and support services.
• May be involved in discharge planning/transitioning patients to their homes.

Skills and Qualifications:

• Registered Nurse License (RN): Current and valid RN license in the state of CA.
• Clinical Skills: conducting assessments, implementing care plans, patient status.
• Communication Skills: interpersonal & communication skills to effectively interact with patients, families, and other healthcare professionals.
• Organizational Skills: manage a caseload, coordinate care, and prioritize tasks.
• Problem-Solving & Decision-Making: Ability to analyze situations, identify problems, and make sound decisions in a timely manner.
• Knowledge of Home Health Regulations: relevant regulations and standards.
• Experience in Home Health: Experience in home health settings.
• Education: Associate's or Bachelor's degree in Nursing (BSN) preferred.
• Technology: WellSky, Kinnser, Oasis, MS Office Suite

Must be available to work a Fulltime Daytime schedule with some on-call rotations.
