We are seeking an experienced Registered Nurse (RN) to join our skilled nursing team as an MDS Coordinator. This is an excellent opportunity for a detail-oriented clinician who is passionate about resident care, accurate reimbursement, regulatory compliance, and interdisciplinary collaboration.
Responsibilities
- Coordinate and complete the MDS assessment process in accordance with CMS and state regulations.
- Ensure timely and accurate completion of MDS assessments and care area assessments (CAAs).
- Collaborate with the interdisciplinary team to develop and update individualized care plans.
- Maximize Medicare, Medicaid, and managed care reimbursement through accurate documentation and coding.
- Review clinical documentation for accuracy and completeness.
- Monitor Quality Measures (QMs) and assist with quality improvement initiatives.
- Maintain compliance with federal, state, and facility regulations.
- Participate in survey preparation and respond to audit requests.
- Educate nursing staff on documentation requirements and MDS processes.
Qualifications
- Current Registered Nurse (RN) license in Connecticut (required).
- Minimum 2 years of MDS Coordinator experience in a skilled nursing or long-term care setting.
- Strong working knowledge of RAI Manual, MDS 3.0, PDPM, Medicare, Medicaid, and CMS regulations.
- Experience with PointClickCare or other electronic health record systems preferred.
- RAC-CT certification is a plus.
- Excellent organizational, analytical, and communication skills.
- Ability to manage multiple deadlines while maintaining accuracy.
