Intake and Assessment Officer (SST)

Pathway Talent is partnering with a leading not-for-profit organisation to recruit two SST Intake & Assessment Officers to support the delivery of the Supporting and Sustaining Tenancies (SST) Program.

The SST program assists people experiencing mental health challenges and housing instability to maintain their tenancies, strengthen independence, and connect with the services and supports they need to thrive.

About the Role
As an SST Intake & Assessment Officer, you will coordinate intake, assessment, referral, and service allocation processes to ensure participants receive the right support at the right time.

You will work closely with participants, community partners, support services, and internal teams to identify needs, assess risks, facilitate referrals, and create pathways to improved wellbeing and housing stability.

This role is ideal for someone who combines strong assessment and administrative capabilities with genuine compassion and a commitment to person-centred practice.

Key Responsibilities

• Conduct intake, eligibility, risk, and comprehensive assessments.
• Review referrals and determine eligibility, urgency, and prioritisation.
• Coordinate participant onboarding and service allocation.
• Facilitate referrals to appropriate internal and external supports.
• Maintain accurate records, databases, case notes, and reporting requirements.
• Build positive relationships with participants, community services, and key stakeholders.
• Promote recovery-oriented and strengths-based approaches to service delivery.
• Contribute to positive housing and wellbeing outcomes for participants.

About You
To be successful, you will be passionate about supporting individuals experiencing mental health challenges, housing instability, or social disadvantage.

• Experience in mental health, homelessness, housing, community services, intake and assessment, service coordination, or case management.
• Knowledge of recovery-oriented and person-centred practice.
• Strong communication, engagement, and relationship-building skills.
• Excellent organisational, administrative, and time-management abilities.
• The ability to manage competing priorities while maintaining attention to detail.
• A collaborative approach and commitment to achieving positive participant outcomes.
• Qualifications in Community Services, Mental Health, Social Work, Psychology, Human Services, or a related discipline will be highly regarded.
