About the Opportunity:
- Schedule: Sunday to Thursday
- Hours: 9am to 5pm (1 day a week will be 10am to 6pm)
- Setting: Permanent Housing facility
Responsibilities:
- Providing case management to the assigned caseload
- Developing and monitoring individual support plans
- Assisting with problem-solving and crisis intervention
- Accompanying consumers on appointments
- Supporting facilities management and maintenance standards
- Performing other duties, as needed
Qualifications:
- 3 years of experience in Behavioral Health
- High School Diploma / GED
- Proficiency in Microsoft Office
- Ability to write case notes
- Strong communication skills
- Punctuality and reliability
- Team player mentality
- Empathy and professionalism
Desired Qualifications:
- 3 years of experience with the target population
- Bachelor’s Degree
- Experience in Peer Counseling or Advocacy
- Substance Use and Mental Health knowledge
- Proficiency in Case Management
