Collarcare is partnering with a well-regarded aged care provider on the Central Coast, NSW to recruit an experienced and compassionate Admissions Registered Nurse for a permanent full-time position.

About the Role
As the Admissions Registered Nurse, you will lead and coordinate the admissions process, ensuring incoming residents receive high-quality, person-centred clinical care from pre-admission through to onboarding.
Working closely with the Facility Manager, Clinical Care Coordinator, residents, families, and multidisciplinary teams, you will oversee assessments, care planning, documentation, and clinical communication to support outstanding resident outcomes.
Key Responsibilities
Coordinate and manage resident admissions and onboarding processes
Complete comprehensive clinical assessments, care plans, evaluations, and admission documentation
Ensure all documentation, reports, progress notes, and care records are accurate and completed within required timeframes
Support and guide residents and families throughout the admissions journey
Build strong working relationships with residents, representatives, and clinical teams
Coordinate clinical care requirements including:
Medication management
Wound care
Pain management
Nutrition & hydration
Behaviour support planning
Continence management
Collaborate with multidisciplinary teams to ensure safe, high-quality care delivery
Participate in audits, accreditation preparation, quality improvement initiatives, and compliance activities
Escalate clinical concerns, incidents, feedback, and complaints in line with organisational procedures
About You
To be successful in this role, you will have:
✔ Current AHPRA Registration as a Registered Nurse
✔ Previous aged care nursing experience
✔ Strong understanding of clinical assessments, admissions, and care planning
✔ Excellent communication and stakeholder engagement skills
✔ Strong organisational skills and attention to detail
✔ Confidence managing documentation and clinical compliance requirements
✔ A collaborative and resident-centred approach to care
✔ Ability to work effectively within multidisciplinary teams
