About the Role As the Clinical Care Coordinator, you will play a key leadership role in supporting the clinical team and ensuring the highest standards of resident care.
Working closely with the Facility Manager, Registered Nurses and Care Staff, you will oversee clinical operations, provide leadership and mentoring, and ensure compliance with aged care standards while promoting continuous improvement.
Key Responsibilities
  • Coordinate and oversee the delivery of high-quality, person-centred clinical care.
  • Review, implement and monitor resident care plans.
  • Provide clinical leadership, mentoring and support to Registered Nurses and Care Staff.
  • Respond to complex clinical situations and provide expert nursing guidance.
  • Monitor residents' health, clinical outcomes and ongoing care needs.
  • Ensure clinical documentation is accurate, compliant and completed to a high standard.
  • Liaise with General Practitioners, hospitals, allied health professionals and external service providers.
  • Drive continuous improvement initiatives and maintain accreditation readiness.
  • Foster a positive, collaborative and supportive team culture.
About You To be successful in this role, you will have:
  • Current AHPRA registration as a Registered Nurse.
  • Minimum 5 years' experience in aged care or a related healthcare environment.
  • Previous leadership or senior clinical experience.
  • Strong knowledge of the Aged Care Quality Standards and clinical governance.
  • Excellent communication and interpersonal skills.
  • Sound clinical assessment and care planning experience.
  • Strong organisational and problem-solving abilities.
  • A genuine passion for delivering exceptional care to older Australians.
