Wellness Coach (Bilingual)

A community-based residential services organization is seeking a Bilingual Wellness Coach in the Bronx, NY. This role supports residents with case management, community referrals, and day-to-day wellness services in a residential setting. This opportunity is for a Bilingual Wellness Coach, Bronx.

About the Opportunity:

• Shift: Day shift
• Schedule: Sunday through Thursday, with one day from 10:00 AM to 6:00 PM
• Hours: 9:00 AM to 5:00 PM
• Setting: Onsite

Responsibilities:

• Provide case management support to an assigned caseload of residents
• Assist clients with access to medical, behavioral health, substance use, vocational, food, clothing, and public benefit services
• Conduct follow-up, document services, and maintain accurate case notes in compliance with agency standards
• Accompany residents to appointments and support continuity of care through referrals and coordination with service providers
• Support daily living skills, community integration, and other resident needs in a residential program setting

Qualifications:

• Bachelor’s degree
• 3+ years of experience working with a behavioral health population
• Working knowledge of email, Microsoft Word, Excel, PowerPoint, and Windows operating systems

Desired Skills:

• Bilingual English/Spanish communication skills
• Experience with substance use, mental health, or homelessness services
• Experience in group work, tenant organization, or community outreach
• Experience working in a residential or supportive housing setting
• Strong interpersonal skills, professionalism, punctuality, and reliability
