Home Care Coordinator/Case Manager

We are seeking an organized, dependable, and compassionate Home Care Coordinator to join a growing home care team in Erie, PA. This position is responsible for coordinating caregivers and client services, managing schedules, handling day-to-day issues, and helping ensure clients receive reliable, quality care in their homes. The ideal candidate is a strong communicator and quick problem-solver who is comfortable taking initiative, managing multiple priorities, and working with clients, families, and caregivers in a fast-paced environment.

Responsibilities

• Coordinate caregiver schedules based on client needs, availability, and location

• Manage schedule changes, call-offs, and other staffing issues as they arise

• Communicate regularly with clients, families, caregivers, and service coordinators

• Help address client concerns and resolve problems quickly and professionally

• Monitor client needs and assist with updates or changes to services

• Assist with recruiting, onboarding, and training caregivers

• Maintain accurate documentation and required client/caregiver information

• Help ensure services remain compliant with agency and Pennsylvania requirements

• Travel locally as needed to support clients and caregivers

Qualifications and Skills

• Previous management, supervisory, or team leadership experience required

• Home care, healthcare, caregiving, case management, or similar experience strongly preferred

• Scheduling or staff coordination experience preferred

• Strong communication and phone skills

• Excellent organization and attention to detail

• Ability to think quickly and problem-solve when unexpected situations arise

• Comfortable managing difficult situations and client concerns

• Self-motivated, dependable, and able to take initiative

• Comfortable using computers and learning scheduling/documentation systems

• Must have a valid driver's license and reliable personal vehicle

• Ability to obtain required PA clearances/background checks and TB testing

Benefits/Compensation

• Full Time

• First shift (8am – 5pm)  

• $18+/hr based on experience

• Temp to hire position  

• Weekly pay while employed through Career Concepts
