Local Community Connector

Chandler Macleod is recruiting on behalf of our client for multiple APS4 Local Community Connector opportunities across Western Australia. This is an exciting 12-month assignment supporting people with disability, their families, carers, and communities to access information, services, and supports.

Working within a community-focused environment, you will play a key role in building relationships, promoting inclusion, and supporting individuals to connect with the National Disability Insurance Scheme (NDIS) and broader community services.

About the Role As a Local Community Connector, you will provide a visible, local presence within the community and support people to navigate available services and pathways. You will work collaboratively with participants, service providers, community groups, and internal stakeholders to promote participation, inclusion, and positive outcomes.

Key Responsibilities

• Provide information about disability supports, community services, and available programs
• Support participants and service providers by gathering and verifying information
• Assist with participant plan implementation, monitoring, and ongoing engagement
• Develop and maintain strong relationships with local community stakeholders
• Promote inclusive and culturally safe community participation
• Conduct participant check-ins and support evidence gathering activities
• Complete administrative tasks and maintain accurate records
• Participate in outreach activities within regional and remote communities

About You To be successful, you will demonstrate:

Customer Service & Community Engagement

• Experience delivering high-quality customer service within community, government, health, or service-focused environments
• Ability to respond to enquiries, resolve issues, and provide accurate information
• Strong communication and stakeholder engagement skills

ICT Capability

• Experience using databases, case management systems, and Microsoft Office applications
• Ability to maintain accurate records and support reporting activities
• Confidence working across multiple digital systems

First Nations Cultural Capability

• Understanding of Aboriginal and Torres Strait Islander cultures, histories, and communities
• Demonstrated ability to engage respectfully and effectively with First Nations peoples
• Experience contributing to culturally safe and inclusive environments

Role Requirements

• Ability to travel within regional and remote communities, including overnight stays
• Current driver's licence and willingness to drive long distances when required
• Comfortable undertaking community outreach, face-to-face engagement, and phone-based support
• Ability to work in office, community, and occasional work-from-home environments
• Capacity to navigate varied locations and terrains as part of community engagement activities
