An exciting opportunity to join a value-based care organization based in Arizona. The practice is seeking a board-certified Nurse Practitioner in Primary Care/ Adult Medicine or Geriatrics experience to join their team.

The organization operates under a value-based care model emphasizing preventive medicine, chronic disease management, care coordination, and quality performance metrics. Physicians practice in an outpatient setting supported by interdisciplinary clinical staff, with structured workflows designed to support patient panel management and continuity of care.

• M-F
• Traveling role seeing patients in their homes
• Geriatric Population, experience preferred but not required
• No Nights, No Weekends.
