The Client (Patient) Care Manager plays a pivotal role in overseeing and managing the client care of patients, ensuring that their needs are met with compassion and efficiency. You will serve as a primary point of contact for both patients and caregivers, actively resolving any issues that arise and facilitating effective communication between all parties involved.
Responsibilities:
- Conducts initial patient screenings and develops patient care plans.
- Resolves patient and caregiver complaints and other issues.
- Travels to patient residences to conduct investigations of issues as needed.
- Responsible for monitoring Patient's off schedule for hospitalization or other reasons and coordinating with scheduling department.
- Responds to changes in patient condition reported by caregivers and nurses.
- Participates in the quality assurance reviews and evaluation of agency services.
- Assist with answering incoming phone calls and talking to potential employees and patients.
- Assist with interviewing caregiver applicants and conducting orientation for them.
- Assist with scheduling patient visits.
- Collaborate with healthcare providers and community resources for comprehensive patient care.
- When on call, take calls after hours emergencies, scheduling issues and client referrals/intakes.
- Learns our computer systems for personalized care plans & communication.
- Your a liaison to provide connection with the social service delivery services locally.
- Case management service to follow up on service delivery.
- Do quarterly assessments & documents Patient process & update assessments.
- Education and Experience
Associate's degree (A. A.) or equivalent from two-year college or technical school preferred.
3+ years related experience or training in healthcare operations
Caregiving, nursing, or home care experience is a big plus.
Must have intermediate computer skills and be comfortable with Wellsky.

Salary: $75,000- $87,500 Annually
Bonus Plan: Paid every quarter
