About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday
- Hours: 8am to 5pm
- Setting: Adult Day Health Center / Rehab Gym
Responsibilities:
- Evaluating mobility and participant needs
- Completing PT assessments and reassessments
- Developing individualized care plans
- Providing various therapeutic exercises
- Collaborating with a multidisciplinary team
- Performing other duties, as needed
Qualifications:
- Master's Degree
- Active CA Physical Therapist (PT) license
- CPR certification
- Working knowledge of Electronic Medical Records (EMRs)
- Solid assessment and documentation skills
- Ability to work collaboratively
Desired Qualifications:
- Experience working with the Geriatric population
- Experience working within a Skilled Nursing Facility
- Experience working within an Assisted Living and/or Rehab setting
- Experience using of Mechanical Assist Devices
