Occupational Therapist — Hayward, CA 
Permanent Placement | $60–$65 per hour

Kace Premier Medical Talent is recruiting an Occupational Therapist for a skilled nursing rehabilitation opportunity at a hospital site in Hayward, California  Help patients regain practical skills and improve independence through individualized occupational therapy.

Responsibilities:
• Assess patients’ ability to perform daily living activities.
• Develop treatment plans addressing functional limitations and rehabilitation goals.
• Provide self-care training, therapeutic activities, and appropriate functional interventions.
• Assess adaptive equipment needs and teach safe, effective strategies.
• Educate patients and caregivers.
• Collaborate with the interdisciplinary team on care and discharge planning.
• Reassess progress and complete accurate clinical documentation.

Qualifications:
• Graduate of an accredited occupational therapy program.
• Current California OT license, or eligibility to obtain licensure before starting.
• Strong assessment, clinical reasoning, and documentation skills.
• Ability to collaborate effectively across disciplines.
