·       Are case logs needed? If so, what are the requirements?  yes

 
Hospital/unit questions

·       Number of ER Bays? 28

·       Number of Patients/Hour? 54,000 ER visits last year

·       What’s the patient mix? Normal for sole community hospital

·       Trauma center designation? Level 3

·       Is there nurse/APP Triage for lower acuity? Yes

·       Shift lengths? (8, 10, 12)

·       AM and PM Shift Coverage Needed?

·       Staffing Model: How many other EM Providers are on at the same time?

·       What does support staff include? Normal ER support staff

·       ED Admission Rate? 15%
