Registered Nurse - Community Clinical Nurse (Home Care) Port Macquarie | Full-Time

Collarcare is partnering with a growing, values-driven home care provider seeking an experienced Community Clinical Nurse to join their expanding team. This is an exciting opportunity to work across the community, supporting older Australians to remain safely and independently in their own homes while being part of an organisation committed to delivering exceptional, person-centred care.

About the Role This is a diverse clinical role where no two days are the same. You will work closely with clients, families, GPs, allied health professionals and the wider care team to ensure exceptional clinical outcomes.
Your responsibilities will include:
  • Completing comprehensive clinical assessments for new and existing clients.
  • Developing, implementing and reviewing individualised care plans.
  • Working collaboratively with Case Managers to coordinate holistic care.
  • Liaising with General Practitioners, Specialists, Allied Health professionals and family members.
  • Conducting post-hospital and incident reviews.
  • Monitoring chronic health conditions and providing ongoing clinical support.
  • Delivering advanced nursing care including wound management, catheter care, medication support, dementia care and other complex clinical interventions.
  • Promoting best practice infection prevention and control.
  • Providing clinical leadership, mentoring and guidance to care staff.
  • Supporting quality improvement initiatives and compliance with the Aged Care Quality Standards.
  • Assisting with clinical governance, mandatory reporting (including SIRS), audits and National Quality Indicators.
  • Stepping into care coordination and case management when required to ensure continuity of care.
  • Working closely with senior leadership to continually improve clinical service delivery.
About You

We're looking for someone who is clinically confident, compassionate and passionate about delivering outstanding care.

You will have:
  • Current AHPRA registration as a Registered Nurse.
  • Current National Police Check.
  • Current Australian Driver Licence.
  • Experience working within Home Care, Community Nursing or Residential Aged Care.
  • Strong knowledge of the Aged Care Quality Standards.
  • Understanding of SIRS, clinical governance and aged care legislation.
  • Excellent communication and relationship-building skills.
  • Strong assessment and clinical decision-making abilities.
  • The ability to work independently while remaining a collaborative team member.
  • A genuine passion for improving the lives of older Australians.
