What You'll Do
- Coordinate and complete accurate, timely MDS assessments in accordance with Medicare, Medicaid, and state regulations.
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
- Ensure accurate documentation to support optimal reimbursement and regulatory compliance.
- Monitor quality measures, identify opportunities for improvement, and implement action plans to enhance clinical outcomes.
- Review resident records to ensure assessments accurately reflect residents' clinical conditions.
- Provide education and guidance to nursing staff regarding MDS documentation and care planning.
- Stay current on CMS regulations, PDPM guidelines, and industry best practices.
- Collaborate with leadership to prepare for surveys and maintain survey readiness.
- Participate in quality assurance and performance improvement (QAPI) initiatives.
What We're Looking For
- Current RN or LPN license in good standing in Ohio.
- Previous MDS Coordinator experience in a skilled nursing or long-term care setting preferred.
- Strong knowledge of MDS 3.0, PDPM, Medicare, Medicaid, and CMS regulations.
- Excellent assessment, organizational, and communication skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- RAC-CT certification is a plus but not required.
