MDS Coordinator

What You'll Do

• Coordinate and complete accurate, timely MDS assessments in accordance with Medicare, Medicaid, and state regulations.
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
• Ensure accurate documentation to support optimal reimbursement and regulatory compliance.
• Monitor quality measures, identify opportunities for improvement, and implement action plans to enhance clinical outcomes.
• Review resident records to ensure assessments accurately reflect residents' clinical conditions.
• Provide education and guidance to nursing staff regarding MDS documentation and care planning.
• Stay current on CMS regulations, PDPM guidelines, and industry best practices.
• Collaborate with leadership to prepare for surveys and maintain survey readiness.
• Participate in quality assurance and performance improvement (QAPI) initiatives.

What We're Looking For

• Current RN or LPN license in good standing in Ohio.
• Previous MDS Coordinator experience in a skilled nursing or long-term care setting preferred.
• Strong knowledge of MDS 3.0, PDPM, Medicare, Medicaid, and CMS regulations.
• Excellent assessment, organizational, and communication skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• RAC-CT certification is a plus but not required.
