Position Overview:
Aequor Healthcare is seeking a Licensed Practical Nurse (LPN) to work at Glen Hazel-Kane Community Living Center. This position offers the opportunity to provide quality nursing care in a long-term care setting while supporting residents' medical needs and overall well-being.
Responsibilities:
• Administer medications and treatments as ordered
• Monitor resident conditions and report changes
• Perform nursing assessments within scope of practice
• Maintain accurate documentation and medication records
• Assist with care planning and resident support
• Collaborate with RNs, providers, and interdisciplinary team members
• Respond to resident needs and emergencies as appropriate
Requirements:
• Active LPN license
• Minimum 1 year of LPN experience preferred
• Must be able to pass background screening
