A nonprofit services organization in New York City is seeking a Licensed Practical Nurse (LPN) to join their Preventive Services Program as a Case Manager.

About the Opportunity:
  • Schedule: Full-time
  • Hours: 9am to 5pm or 10am to 6pm or 11am to 7pm
Responsibilities:
  • Conducting family-based home visits
  • Assessing health concerns and providing education/support
  • Helping families understand medical recommendations and follow-up care
  • Serving as a liaison between families, hospitals, clinics, and community providers
  • Coordinating care with social workers, program leadership, and medical providers
  • Supporting preventive health, chronic illness management, maternal/child health, and family health education
  • Completing documentation and follow-up as needed
Qualifications:
  • Graduate of an accredited School of Practical Nursing
  • Active NYS Licensed Practical Nurse (LPN)
  • Solid assessment, clinical, and documentation skills
