What skills are required: Inpatient hospital pharmacy at least 1 year, IV experience

Dates of Coverage needed: Nov. 30th latest thru 3+ months with likelihood of extension

Coverage Hours needed: opened 7 days a week , 7-330pm. Need 40 hours/week coverage, with rotating schedule to include weekends. There is remote pharmacy services during closed hours

Is weekend coverage needed? yes, weekend rotation, same hours (possibly reduced hours in future on weekends)

Will this person need to take on-call? Yes, rotating , during closed hours

Staff Size: 1 DOP, 2 RPh, 2 Tech

Job responsibilities: Verify orders (CPOE), process orders, dispense controlled substances, check tech’s work, attend IDT, vancomycin dosing, monitor renally cleared drugs and hepatic cleared drugs, purchasing oversight (Cardinal Order Express).

Tech’s role: Techs manage Pyxis refills, expiring drugs, unit inspections, fill and check clinic (3) stock, purchasing order entry, reconciling charges, 340B audits, reconcile coding variances, order supplies.

How many days of training/orientation are required prior to start? Need to start with HR’s orientation, then on the job training at least 1 week overlap (possibly more)

Dress Code: Scrubs or casual (sport shirt, polo, no jeans, casual slacks)

Additional Information: Possible lodging option: Haven Gallup (furnished apartment)
