The Role:
- Working with the frail aged and people with a disability to maximise independence at home
- Working with clients, carers and families to provide practical solutions to promote independence and safety at home
- Developing goals and care plans to assist clients to remain at home and active in their local community
- Collaborating with other clinicians in the RAHT team to offer holistic care to clients referred
- Working with a diverse range of clients and health conditions.
- Working across funding streams such as HACC PYP, CHSP & possibly the NDIS (minimal, if any)
- Opportunity to work with Allied Health Assistants to undertake client care programs and equipment trials.
- Primarily home based work with some opportunity for clinic appts.
About You:
- Community OT experience
- Knowledge of Australian Building Standards with Home modifications
- Experience with Scooter assessments, Wheelchair prescription
- Strong communication and interpersonal skills with a pleasant, kind manner
- Current, valid AHPRA Registrations
- Have a valid Background/ Police Check, plus are able to provide at least 2 working references
- Be comfortable to work in regional setting
- Familiar with funding schemes such as Geat2GO and SWEP is desirable
- Working with the elderly is a bonus!
