Facility Name and address: Great Falls Clinic Hospital, 3010 15th Ave S, Great Falls, MT 59405
Licensed Beds:45 Average Daily Census: 25-30
Position: Interim Director of Pharmacy and Pharmacist in Charge (PIC) – over Clinic, Infusion Center and HOPD
What skills are required: Management, inpatient pharmacy, leadership
Is specific state licensure required? Montana
Dates of Coverage needed: ASAP until a perm is hired (likely 3-4+ months)
Pharmacy Hours Open: The hospital pharmacy is staffed 24/7 because of the needs of the surgery and emergency departments
Coverage Hours needed: This will be fulltime (40 hours/week), M-F daytime coverage (can be flexible but mostly 8am-5pm with unpaid lunch break or similar)
Will this person need to take on-call afterhours? Not likely
Order Entry System: Meditech - can train
Site Automation: Touchpoint
Site Specialty Areas: PACU, HOPD, and the clinic hospital. The hospital functions like a surgical hospital but has a busy ED and an infusion center.
Staff Size: The clinic hospital is staffed with 6 pharmacists and 4 technicians. The infusion center is staffed with 3 pharmacists and 3 technicians. One of the pharmacists splits time working at the anticoagulation clinic.
Job Responsibilities: Will be DOP and will need to hold PIC. There will be a mix of administrative and staffing responsibilities. Leadership is needed to stabilize account.
Will there be sterile compounding involved, or could sterile compounding potentially be a part of their job function? Yes
Upcoming surveys: They are in the window for an annual DNV survey.
HR Requirements (see attached and below): (Hannah will manage once we have a candidate)
· Online verification of license to include expiration date and any disciplinary actions – within 30 days of start
· Vaccination Records:
· MMR – 2 vaccine doses or positive titers
· Varicella – 2 vaccine doses, positive titer, or documented history of infection
· Hepatitis B – vaccine series, titer, or declination
· Covid-19 – vaccine doses, or declination
· Flu Vaccination – required October 1st through the end of flu season – must be from THIS flu season – declination accepted on condition traveler wears mask through flu season.
· TDAP – proof of vaccination including Pertussis within the past 10 years
· Two PPDs or QuantiFERON/TSpot/TB Gold with negative results within 12 months of start date (needed prior to start)
· Positive PPD will require clear chest x-ray within 5 years and TB Questionnaire within one year of start date
· 10 Panel Drug Screen – within 6 months of start date
· Fit Test – N95 mask – within 12 months of start date:
· Document should indicate if employee has good fit seal check with mask
· This step can be completed upon arrival. An appointment with employe health can be arranged.
· Statewide Criminal Background check – within 6 months of start date – must go back 7 years for all states lived and worked to include all aliases
· GSA/EPLS – within 30 days of start date
· OIG – within 30 days of start date
