Clinical and Care Partnering Manager - Cairns

The Clinical Care Case Manager is responsible for the management and delivery of clinical support to teams across CHSP, HACC and HCP within a Consumer Directed Care framework.

• The Clinical Case Manager will focus on delivering support and leadership to the Home Care team across Cairns region
• The organisation will contribute to developing you into a Service Management position
• $140,000 + Super + Salary Packaging

About the role:
The Community Home Care Clinical Case Manager will help to ensure high standards of the services are person centered, responsive and support clients to live as well and independently as is possible in their own home and community based across the Cairns region. To help achieve this purpose, the Clinical Case Manager will provide effective support and leadership to the team and focused on delivering high-quality, person-centred care and strong clinical governance across the region.
Duties:

• Lead the Clinical team to manage CHSP, HACC and HCP programs to ensure appropriate services are delivered that meet client needs.
• Ensure services meet Aged Care Quality Standards, legislative requirements and organisational policies
• Lead clinical risk management, including incident oversight, investigation and continuous improvement actions
• Monitor clinical governance frameworks and implement strategies to enhance safety and quality of care
• Conduct audits, reviews and reporting to support compliance and accreditation
• Develop a continuous improvement culture and mentor Registered Nurse’s in the resolution of client care issues and problems.
• Oversee practice in clinical assessment, care planning and ongoing reviews aligned to client goals and preferences
• Support best practice clinical interventions including chronic disease management, wound care and palliative care
• Identify and resolve clinical issues as they arise.

Skills and experience:

• Current AHPRA registration as a Registered Nurse
• Demonstrated leadership experience in community or aged care services
• Strong understanding of Aged Care Quality Standards and regulatory requirements
• Experience in clinical governance, risk management and quality improvement
