JOB DESCRIPTION

Weeks Medical Center - Colebrook NH outpatient clinic

Locum opening for APRN, PA, MD, or DO

Preferred rates: APP $160/hr - Physicians $215/hr

• Provider must have experience in outpatient primary care and comfort working autonomously
• Well experienced in chronic disease management and able to have difficult conversations with patients regarding medication management and patient education
• RHC in Colebrook NH - recently renovated office building across from UCVH.
• Position to start ASAP for a (3) three month duration, with possibility of extension until permanent needs are met
• NH licensed candidates are preferred due to full payor enrollment and credentialing timeframe. Temp licenses are not accepted. Position is for replacement of provider leaving - Full payor enrollment required.

Primary Care:

• Family Practice - pediatric through geriatric (Internal Medicine is also acceptable)
• High elderly population with multiple comorbidities
• Monday through Friday 8a-5p (12-1 lunch) - would consider 4 days per week for the right candidate
• 36 patient care hours, 4 hours of admin built into schedule (not eligible for overtime, unless approved in advance)
• 16+ patients per day, concentration on chronic disease management
• EMR is Meditech Expanse, Dragon dictation
• Supported by practice manager, nursing staff, and designated MA
