In this role, the Case Manager will be responsible for dealing with clients facing homelessness and potentially mental health or substance abuse issues.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 9am to 5pm (one late night a week = 11am to 7pm)
- Setting: Transitional Housing
Responsibilities:
- Providing direct services to a caseload of approximately 15 clients
- Conducting psychosocial evaluations
- Connecting clients to psychiatric and medical services
- Obtaining identification and benefits for clients
- Meeting weekly with clients to develop service plans
- Performing other duties, as needed
Qualifications:
- Bachelor’s Degree
- Experience in a Shelter, Mental Health, or Substance Abuse setting
- Experience with Harm Reduction techniques
- Motivational interviewing skills
- Proficient in Microsoft Word and Excel
- Excellent writing and communication skills
Desired Qualifications:
- Bilingual (English and Spanish)
- Experience with AWARDS/Cares systems
- Motivational Interviewing techniques
