Position Highlights:

- Doctoral degree in Psychology is required (Ph.D or Psy.D)
- Bilingual in English and Spanish is preferred, but not required
- Current valid CA license required
Responsibilities:
- Conduct Annual Health Assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
- Perform evidence-based practice screenings including point of care testing (as appropriate)
- Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
- Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of the assessment
- Communicate findings in the assessment that will be used to inform patients primary care provider of potential gaps in care
- Identify urgent and emergent situations and intervene appropriately
- Educate patients on topics such as disease process, medication, and compliance
Compensation/Benefits:
- Salary starts at $105k/year plus bonus
- Salaries and are dependent on knowledge, skills and experience
- The base pay does not include other forms of compensation and/or incentives
