Bilingual Nurse Practitioner – Primary Care

The organization delivers family medicine, internal medicine, pediatric and geriatric care and sponsors an established Family Medicine Residency Program. This is an excellent opportunity for a bilingual Nurse Practitioner who values collaborative care, patient education and meaningful service to the community.

Responsibilities

• Assess patients and obtain relevant medical, medication, allergy, psychosocial and family histories
• Diagnose and manage acute illnesses, chronic conditions and episodic exacerbations
• Review abnormal laboratory results and determine appropriate treatment or follow-up
• Develop individualized care plans based on clinical findings and patient priorities
• Provide clear, culturally sensitive education regarding wellness, illness, treatment and rehabilitation
• Identify the need for additional services, patient education or specialty referrals
• Collaborate with physicians, case managers, family-planning professionals and other members of the care team
• Involve family members or significant others in the care plan when appropriate
• Deliver care in accordance with approved clinical protocols and established standards of practice
• Evaluate patients’ responses to treatment and adjust care plans as needed
• Document education, treatment recommendations, patient understanding and follow-up needs
• Serve as a clinical resource for healthcare team members
• Participate in quality-improvement initiatives and support changes that improve patient care
• Assist with the orientation and education of new employees or students when requested
• Help develop and maintain patient and staff educational resources
• Travel between nearby clinical locations as needed

Qualifications

• Licensed or eligible for licensure as a Nurse Practitioner in Massachusetts
• At least two years of recent Nurse Practitioner experience
• Fluency in both Spanish and English required
• Ability to work effectively within a multidisciplinary, team-based environment
• Strong clinical assessment, communication and patient-education skills
• Willingness to learn from and share knowledge with colleagues
• Commitment to culturally responsive, community-based healthcare
