Experienced Community Occupational Therapist required for this regional hospital, just two hours from Melbourne. Ideally you will be an experienced clinician who is able to confidently carry a caseload and offer support to the junior OTs on the team. There is a 6 month contract or permanent role available. Ideally to start in March/April. Accommodation provided for the duration of the contract. Open to experienced Grade 1 clinicians looking to take a step up.
The caseload involves:
- Working with the frail aged and people with a disability to maximise independence at home.
- Working with clients, carers and families to provide practical solutions to promote independence and safety at home
- Developing goals and care plans to assist clients to remain at home and active in their local community
- Collaborating with other clinicians in the RAHT team to offer holistic care to clients referred
- Working with a diverse range of clients and health conditions.
- Working across funding streams such as HACC PYP, CHSP & possibly the NDIS (minimal, if any)
- Opportunity to work with Allied Health Assistants to undertake client care programs and equipment trials.
- Primarily home based work with some opportunity for clinic appts.
Ideally you will have experience across:
- Home modifications, knowledge of Australian Building Standards
- Assistive technology/equipment prescription
- Scooter assessments, Wheelchair prescription
- Familiar with funding schemes such as Geat2GO and SWEP
- Community
