Job Title: Medical Social Worker Location: Limerick Region Contract: Full-Time, Permanent
An established transitional care and rehabilitation service in the Limerick region is seeking a Medical Social Worker to join its multidisciplinary team. The role involves supporting patients transferred from acute hospitals who require ongoing rehabilitation, discharge planning, and community support prior to returning home.
Key Responsibilities • Provide psychosocial assessments, counselling, and emotional support to patients and families • Assist with discharge planning and coordination of community services • Work collaboratively within a multidisciplinary team to support patient-centred care and SMART goal planning • Develop and maintain comprehensive Social Work care plans and clinical records • Liaise with hospitals, community agencies, and external services as required • Participate in MDT meetings, peer supervision, and service development initiatives
Requirements • Qualification in Social Work • CORU registration essential • Strong communication, interpersonal, and advocacy skills • Previous healthcare, rehabilitation, or medical social work experience desirable
