Overview
A Medical Social Worker is required to support patients and families within a rehabilitation setting, ensuring appropriate supports are in place to facilitate safe discharge and continuity of care.
Key Responsibilities
- Complete psychosocial assessments
- Support discharge planning and care coordination
- Liaise with families, community services, and external agencies
- Provide advocacy and emotional support
- Assist with complex discharge planning, referrals, and service access
Requirements
- Qualification in Social Work
- CORU registration
- Experience in a medical, hospital, or rehabilitation setting
- Strong knowledge of community services and discharge pathways in Ireland
