Apply directThis is a Physical Therapist opportunity in the home health industry with a very reputable and established hospice and home health company. The ideal candidate is a skilled professional that is passionate about delivering quality care and making a real difference.
Responsibilities
- Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
- Assessing patient mobility, developing individualized treatment plans, and providing in-home therapy to improve function, manage pain, and prevent falls.
- Implement therapeutic exercises, use manual therapies, and educate patients and their families on exercises and equipment to promote independence and a higher quality of life within the home environment.
- Prepare and submit clinical and progress summaries based on identified and attained goals.
Recent graduates with a state license will be considered.
Qualifications
- Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum.
- Certification in Physical Therapy and valid state license to practice.

Benefits & What we are Offering.

- We Will assist/contribute to relocation services costs.
- Estimated Total Annual Income is $100k-$180k Depending on experience.
- Compensation/Pay Scale includes additional “pay per visit” income.
- Benefits (Medical, Dental, Vision) company contributes $475/month to each employee to apply towards healthcare plan cost (several plans to choose from).
- 401K contribution is currently a10% match of whatever the employee contributes annually.
- PTO- 2 weeks the first year and then 3 weeks after 2 yrs of employment.
