Intake Coordinator

As a Home Care Intake Coordinator you will manage the patient admission process, acting as the first point of contact to gather patient info, verify insurance/benefits, evaluate service appropriateness, and coordinate with clinical teams for smooth transitions to home care, requiring strong communication, organization, and knowledge of home health regulations. Key duties include managing calls, completing paperwork, coordinating with hospitals/physicians, documenting everything in systems, and ensuring regulatory compliance for clients.

Key Responsibilities

  • Referral Management: Receive, process, and log all incoming referrals from hospitals, physicians, and facilities.
  • Patient/Family Communication: Answer inquiries, explain services, and address concerns from patients, families, and referral sources.
  • Information Gathering: Collect essential demographic, medical, and insurance information.
    Eligibility & Benefits: Verify insurance benefits, negotiate rates, and obtain initial authorizations for services.
  • Care Coordination: Collaborate with hospital case managers, physicians, and the internal clinical team (nurses, therapists) for seamless discharges and admissions.
  • Documentation: Accurately enter patient data, referral status, and follow-ups into the agency's software system (WellSky).
  • Compliance: Ensure all admissions and documentation meet state, federal, and agency regulations (like Medicare guidelines).
  • Essential Skills & Qualifications

Experience:

  • Previous experience in home health, hospice, home care or a related clinical/administrative setting is preferred.

Knowledge:

Understanding of home health/home care regulations and insurance verification processes.

Communication:

Excellent verbal and written skills; ability to listen actively and communicate with empathy.

Organization:

  • Strong attention to detail, ability to multitask, and manage a high volume of information.
  • Technical Skills: Proficiency with computer systems and databases.
